Connecting Clinical Diagnosis to ICD-10 Codes

Defending medical necessity always starts with clear and complete documentation. Too often, chiropractors confuse the written diagnosis with the billing code — but these are not the same! You need to know how they connect!
A strong clinical record clearly shows how you arrive at each diagnosis and how it directly supports the patient’s treatment plan. Because clear records are your best defense, you must show your thought process at every step.

Start With a Clear Clinical Picture

First, begin with the basics:

  • Gather the patient’s full story (History)
  • Combine it with exam findings (Examination)
  • Include any outcomes assessments
  • Use your expertise to create a working or differential diagnosis

So, when you follow this complete process, you lay a solid foundation for compliance and patient care.

Link Your Written Diagnosis to Medical Necessity

Next, you produce a final written diagnosis — and this written diagnosis guides which ICD-10 codes you assign in your EHR. For example, a properly written diagnosis must support your coding choices. If your records are ever audited, they must clearly prove medical necessity. Just like in grade school math, you must “show your work.” Auditors will compare your notes, written diagnosis, and treatment plan to confirm that everything lines up.
